City of Toronto "Notice of Contravention"
More commonly described as a "Backflow preventer warning letter"
The City of Toronto will periodically send out a "Notice of Contravention" to property owners who do not have a Backflow Preventer Test Report on file with the city. In some cases, the recipient of this letter might not even have a backflow preventer installed.

NOTICE OF CONTRAVENTION -- TORONTO MUNICIPAL CODE Section 851 -- WATER SUPPLY
BACKFLOW PREVENTION DEVICE INSTALLATION REQUIRED
A premise isolation prevention device was required to be installed at every water service connection on your property by December 31, 2008. A letter stating this requirement was sent to you in early 2008. Our records indicate that a premise isolation backflow prevention device has not been installed at 123 ANY STREET. This is in contravention of the requirements of Toronto Municipal Code Chapter 851-8 D. Cross connection and prevention. The section states that:
You are now required to have an authorized person (as per Schedule 6, of Municipal Code Chapter 851) install and test the device immediately, and submit an owner-signed City of Toronto designated Test Report Form, available on our website www.toronto.ca/water/backflow. A building permit must be obtained before any work under section 8 of the Municipal Code can commence.
Any installed premise isolation devices must be tested and the owner-signed test report submitted to Toronto Water within 7 days of the test date either by email to [email protected], fax at 416-394-5716, or by mail to Backflow Prevention Program, 30 Dee Avenue, Toronto, ON, M9N 1S9.
Please be advised that failure to comply may result in the City of Toronto exercising its enforcement powers under Toronto Municipal Code Chapter 851, as applicable.
